Vzniká soutokem horských řek Vydry a Křemelné pod Čeňkovou pilou na Šumavě. Za hlavní pramenný tok se považuje Vydra s vlastním povodím 147 km2. Zpočátku, od soutoku k Sušici, Otava teče severním směrem a poté se až k Horažďovicím ohýbá na severovýchod.
